Attendant Care Program

The Attendant Care Program provides in-home personal care and other support services for physically disabled, mentally alert individuals under the age of 60 in Cambria, Somerset, Blair, Bedford, Huntingdon, Centre, Clearfield, and Fulton Counties. Our Direct Care Workers (DCW) receive comprehensive training to provide in-home services to the recipients of our services, known as consumers.

 

Eligibility for Attendant Care/ACT 150:

  • Ages 18-59
  • PA Resident
  • Able to direct his/her own services
  • Mentally alert and able to direct financial/legal affairs
  • Require Personal Care Assistance with ADL's (Activities of Daily Living) on a regular basis
  • Physical disability that is going to last at least one year
  • Fees may be based on income

 

Eligibility for Attendant Care Waiver:

  • Ages 18-59
  • PA Resident
  • Able to direct his/her own services
  • Mentally alert and able to direct financial/legal affairs
  • Require Personal Care Assistance with ADL’s on a regular basis
  • Determined to have a Level of Care equal to a Nursing Facility Level of Care (determined by AAA - Area Agency on Aging)
  • Eligible for medical assistance

  • Bathing
  • Dressing
  • Transferring
  • Grooming
  • Toileting
  • Light housekeeping
  • Health maintenance activities
  • Personal emergency response systems (PERS)
  • Shopping and errands
