Attendant Care Application Process
- Assessment is scheduled in the individual’s home to complete necessary paperwork.
- An MA-51 Form must be completed by the primary care physician to determine the individual's Level of Care. After the completed MA-51 is received by the SC, the SC will forward to the appropriate Area Agency on Aging so an Initial Options Assessment can be completed by an RN.
- The RN will send the completed Assessment with a Determination Report stating what is determined to be the consumer's Level of Care. If the individual does not need nursing home care they are eligible to start services under the ACT 150 Program. If the individual needs institutionalized in a nursing home facility, he/she will need to apply for Waiver services.
- A PA-600 Form is completed, including financial information. This form is sent to the County Assistance Office for review. The determination on financial eligibility will be sent to the SC.
- If the individual is eligible for the Attendant Care Waiver they will begin services. If the individual is above the income limits for AC Waiver they will begin services on the ACT 150 Program.
- Once this determination is made, the Attendant assignment is completed and an in-home Consumer Orientation is completed by the Service Coordinator to review all program policies and procedures.
- The fee determination for ACT 150 is determined by the Department of Public Welfare Sliding Fee Scale based on family size and monthly gross income.
- There is no fee for services provided in the Attendant Care Waiver.
