COMMCare Waiver

The COMMCare Waiver program is a Home and Community-Based program developed for individuals with a medically determined diagnosis of traumatic brain injury (TBI). COMMCare prevents the institutionalization of individuals with TBI and helps them to remain as independent as possible. Alleghenies United Cerebral Palsy is a service provider for this waiver. Unlimited Care Providers is a subsidiary of Choices which provides service coordination and intake for the COMMCare Waiver. CommCare services are directed and controlled to the maximum extent possible by the consumer.

 

Eligibility:

  • Have a primary diagnosis of Traumatic Brain Injury (TBI) on the MA-51 (medical evaluation) form.
  • Ages 21 or older.
  • PA resident.
  • Nursing Facility Eligible / Special Rehabilitation facility.
  • Substantial functional limitations in 3 or more of the following life activities: Mobility, Communication, Independent Living, Self-Care Behavior, Self-Direction, or Cognitive Capacity.

 

Services Available:

  • Service Coordination through Unlimited Care Providers.
  • Personal Care Services.
  • Respite Care.
  • Environmental Adaptations-$20,000 per lifetime through Unlimited Care Providers.
  • Non-Medical Transportation through Unlimited Care Providers.
  • Specialized Medical Equipment/Supplies & Assistive Technology-$10,000 per lifetime through Unlimited Care Providers.
  • Personal Emergency Response Systems (PERS) through Unlimited Care Providers.
  • Therapies through Unlimited Care Providers.
  • Night Supervision.
  • Community Integration.
