COMMCare Waiver
The COMMCare Waiver program is a Home and Community-Based program developed for individuals with a medically determined diagnosis of traumatic brain injury (TBI). COMMCare prevents the institutionalization of individuals with TBI and helps them to remain as independent as possible. Alleghenies United Cerebral Palsy is a service provider for this waiver. Unlimited Care Providers is a subsidiary of Choices which provides service coordination and intake for the COMMCare Waiver. CommCare services are directed and controlled to the maximum extent possible by the consumer.
Eligibility:
- Have a primary diagnosis of Traumatic Brain Injury (TBI) on the MA-51 (medical evaluation) form.
- Ages 21 or older.
- PA resident.
- Nursing Facility Eligible / Special Rehabilitation facility.
- Substantial functional limitations in 3 or more of the following life activities: Mobility, Communication, Independent Living, Self-Care Behavior, Self-Direction, or Cognitive Capacity.
Services Available:
- Service Coordination through Unlimited Care Providers.
- Personal Care Services.
- Respite Care.
- Environmental Adaptations-$20,000 per lifetime through Unlimited Care Providers.
- Non-Medical Transportation through Unlimited Care Providers.
- Specialized Medical Equipment/Supplies & Assistive Technology-$10,000 per lifetime through Unlimited Care Providers.
- Personal Emergency Response Systems (PERS) through Unlimited Care Providers.
- Therapies through Unlimited Care Providers.
- Night Supervision.
- Community Integration.
