Since 1963 the PA Elks Home Service Program has been helping to improve the quality of life for thousands of children and adults throughout Pennsylvania.

 

Eligibility:

An individual of any age who has a developmental disability. This is a disability that has occurred before a person reaches 22 years of age. It may result in physical or mental delays or a combination of both. If needed, service can continue throughout a person’s lifetime.

 

Services Available:

  • By a registered nurse
  • PA residents
  • In the person's home environment
  • At NO CHARGE

 

An ELKS Home Nurse Can:

  • Provide in-home medical case management services
  • Advocate for an individual's legal and human rights
  • Provide information regarding resources and services available
  • Assess needs and help to access equipment
  • Advocate for appropriate educational services
  • Advocate for appropriate educational services
  • Offer instruction regarding home care
  • Provide emotional support
  • Help with vocational goals
  • Assist with residential placement
  • Provide referrals to appropriate agencies in the community
  • Interpret clinical findings and act as a liaison between health care professionals

Referrals Are Made:

  • By a health care professional
  • By School Personnel
  • By community agencies
  • By a family member
  • By a friend or concerned Elk
