Elks Home Service
Since 1963 the PA Elks Home Service Program has been helping to improve the quality of life for thousands of children and adults throughout Pennsylvania.
Eligibility:
An individual of any age who has a developmental disability. This is a disability that has occurred before a person reaches 22 years of age. It may result in physical or mental delays or a combination of both. If needed, service can continue throughout a person’s lifetime.
Services Available:
- By a registered nurse
- PA residents
- In the person's home environment
- At NO CHARGE
An ELKS Home Nurse Can:
- Provide in-home medical case management services
- Advocate for an individual's legal and human rights
- Provide information regarding resources and services available
- Assess needs and help to access equipment
- Advocate for appropriate educational services
- Advocate for appropriate educational services
- Offer instruction regarding home care
- Provide emotional support
- Help with vocational goals
- Assist with residential placement
- Provide referrals to appropriate agencies in the community
- Interpret clinical findings and act as a liaison between health care professionals
Referrals Are Made:
- By a health care professional
- By School Personnel
- By community agencies
- By a family member
- By a friend or concerned Elk
