The Attendant Care Coordinator serves as the Agency representative in assigned counties and is responsible for working with assigned persons with disabilities to establish independent living plans and goals. The Attendant Care Coordinator determines community services and programs to assist consumers in developing the skills to maximize independence and control of their lives. Caseload available in Altoona.
Minimum of a Bachelor’s Degree in the Human Service or related field from a four-year college or university.
One year of professional human service experience conducting psychosocial assessments preferred.
Supervisory experience a plus.
Requires excellent communication and organizational skills.
