HomeBased Waiver
Home-Based Waiver is a state and county funded program that offers people who choose to live at home with their families the same opportunity for therapeutic supports as those who, in the past, may have needed more intensive residential programs. The intent of this program is to provide supports to individuals outside of licensed facilities, based upon individual need and choice. Supports provided under Home-based Waiver cannot be eligible services paid for via another funding stream like medical insurance or the educational system.
Eligibility:
- Any Age
- PA Resident
- Diagnosis of mental retardation
- Referral/approval from a county MH/MR office
- Financial criteria (Sliding fee scale applies)
- Has a primary caregiver who needs assistance to care for the individual

- Service Coordination
- Community Integration
- Bathing
- Grooming
- Toileting
- Dressing
- Feeding
- Transferring
- Laundry
- Meal Preparation
- Socialization
- Intellectual Stimulation
- One to One supervision
- Therapies as defined and instructed by a therapist
- Accompany to appointments
