Personal Care 0-21Personal Care

Personal care 0-21 program provides services to individuals ages birth to 21 years old that have been diagnosed with a developmental disability and require assistance in all aspects of daily living. To be eligible for this program parents must be working or disabled and not able to care for the child.

 

Eligibility:

  • PA Resident
  • Ages 0-21
  • Diagnosed with a developmental disability
  • Require assistance with daily living
  • Recommended for services by a physician
  • Child of a working parent or primary caregiver is disabled and not able to care for child
  • Medical Assistance (MA) eligibility
  • Approved by PA Department of Public Welfare

Services Provided:

  • Bathing
  • Dressing
  • Range of motion
  • Transfers
  • Bowel and bladder care
  • Meal preparation
  • Feedings, and clean up
  • One-on-one supervision
