Attendant Care

The purpose of the Pennsylvania Attendant Care Program is to enable individuals with physical disabilities to perform activities of daily living. Consumers have ultimate control of their attendant care services including the right to recruit, hire, train, supervise, pay and, if necessary, terminate their attendants.

 

The Attendant Care Program includes the Attendant Care Waiver and the Attendant Care Act 150 Program.  The Consumer Employer (CE) model of service is also an option in this program.  For more information on the CE model and the Fiscal Management Services (FMS), please click here.

 

Community Living Arrangements

Community Living Arrangement (CLA) facilities are for individuals that are physically and mentally challenged. Individuals are offered experiences or activities of daily living, decision making, and social and emotional growth that encourage them to make choices and to take charge of their lives with help available when needed.  Funding is provided by Cambria County Mental Retardation Program and private fees.

 

Community Services Program for Persons with Physical Disabilities

The Community Services Program for Persons with Physical Disabilities (CSPPPD) provides services to people with severe physical disabilities. Individuals must have another related condition, physical disability, or traumatic brain injury. Individuals must experience at least three substantial functional limitations in the areas of mobility, communication, self-care, learning, self-direction, or capacity for independent living.

The CSPPPD program includes the OBRA Waiver, Independence Waiver, and the COMMCare Waiver. The Consumer Employer (CE) model of service is also an option in this program.  For more information on the CE model and the Fiscal Management Services (FMS), please click here.

 

Consumer Employer Model

The Consumer Employer Model is an option for consumers to take total control over their in-home services.  In this model, you, control who is providing your services and when they are provided. The CE Model of Service Provision is an option for services under specific programs regulated by the Dept. of Public Welfare, Office of Long Term Living, Bureau of Home and Community Based Services.

 

Elks Home Service Program

Since 1963, the PA Elks Home Service Program has been improving the quality of life for children and adults throughout Pennsylvania.  The Elks Home Service nurses make home visits free of charge to any individual with a developmental disability.  The Elks Home Service program has been affiliated with Alleghenies UCP for over 37 years.

 

Home Based Waiver

Home-Based Waiver is a state funded program that offers people diagnosed with mental retardation who choose to live at home with their families the same opportunity for therapeutic supports as those who, in the past, may have needed more intensive residential programs.  Alleghenies UCP is a provider of services for these programs.

 

PDA Waiver & PAS

The Pennsylvania Department of Aging (PDA) 60+ Waiver provides home and community based services to eligible person's age 60 or older that are clinically eligible for nursing facility care.  Alleghenies UCP is a provider of services for these programs.

 

Personal Care 0-21

Personal care 0-21 program provides services to individuals ages birth to 21 that have been diagnosed with a development disability and require assistance in all aspects of daily living.

 

Private Pay

Private pay is an option for people who need some of the services that UCP offers without going through the application process for our waiver programs.  A contract for services and payment is completed between the potential consumer and UCP.  Once the contract is completed and agreed upon, services can begin.

 

Transportation

Providing reliable, handicapped accessible transportation through Cambria County MH/MR funded contracts.  Private transportation is also available.
